Period and bleeding disorders

Period and bleeding disorders

Periods that are heavy, irregular or painful, or bleeding between periods, are common and usually treatable. At Evita GyneCare the cause is looked for with an examination, blood tests and a pelvic ultrasound done on site, and the treatment options are explained so you can choose what suits you.

When it’s worth being checked

  • You change a pad or tampon every one to two hours, or pass clots larger than a coin
  • Your period lasts more than seven days
  • Your cycle is shorter than 21 days or longer than 35 days, or periods have stopped
  • Period pain stops you working, studying or sleeping
  • You bleed between periods or after sex
  • You feel tired, breathless or dizzy, which can be a sign of anaemia from blood loss
  • Any bleeding after your periods have stopped for a year — always book promptly

Common causes

Hormonal changes and problems with ovulation (including PCOS and thyroid conditions), fibroids, polyps, adenomyosis, endometriosis, infection, pregnancy-related bleeding, some medicines and, less often, bleeding disorders. Most causes are not serious, but the cause decides the treatment.

How it’s assessed here

  1. History — your cycle pattern, how heavy and painful your periods are, and your plans for pregnancy or contraception.
  2. Examination — explained first, with a female staff member or nurse present. You can decline or stop at any time.
  3. Tests, as indicated — haemoglobin, thyroid, a pregnancy test, and a pelvic ultrasound, usually in the same visit.
  4. Further tests if needed — for example a sample of the womb lining, or referral, depending on your age and findings.

Treatment options

  • Iron to correct anaemia
  • Non-hormonal tablets taken during your period to reduce bleeding and pain
  • Hormonal options such as the pill, progestogen tablets or a hormonal intrauterine device (IUD)
  • Treating the cause, for example thyroid treatment, or referral for fibroid or polyp surgery when that is the right option

Common questions

Can I be examined during my period?

Usually yes. For most period problems there is no need to wait until bleeding stops.

Is period pain normal?

Some cramping is common. Pain that stops you working, studying or sleeping is not something to put up with and is worth investigating.

Will I need surgery?

Most women don’t. Many period problems are treated with tablets or a hormonal device. Surgery is considered only when it is the best option, and you would be referred to a hospital for it.

Can heavy periods cause anaemia?

Yes. Heavy bleeding is one of the most common causes of low haemoglobin in women, which is why a blood count is usually checked.
